Goal Setting Worksheet

Dear Southwood Parents:

Your child has been working on a goal this past term and we thank you for your involvement in helping to set the goals at the parent-teacher conference and in supporting your child in working towards the achievement of this goal. We will be awarding "Notable Achievement" Certificates next week.

A goal at Southwood is for each and every student to be able to strive for excellence. But what EXCELLENCE means for one student my look different for another student. Excellence means that a student demonstrates notable achievement in a certain area through his/her own effort, hard work, and attitude. This effort is most meaningful if supported by his teacher and parents.

Excellence may mean that one student extends her learning in social studies with a project on immigration. Excellence for another student may mean that he focus on improving his skills in reading by reading 10 new books. Another may achieve excellence by showing growth in following directions or behaving better in the classroom.

In order to improve themselves, students must know their strengths as well as those areas that need improvement. We are asking that you take a moment to reflect upon your child and his/her most recent report card and help your child come up with a meaningful goal. If your child demonstrates notable achievement toward this goal, s/he will be recognized at the end of the semester. The teacher will make the final decision as to whether the student achieved his/her goal. Such excellence may also be noted in the narrative comments on the next report card.

Here is the goal that your child would like to work on this term.

Would you like your child to work on a different (or second) goal? If so, what?

_____ I am in support of my student's goal this term.
